The BBC has found that a critical report on the Norfolk and Suffolk NHS trust’s deaths was “watered down” to spare bosses.

That’s exactly what I’d expect to happen when you let people effectively mark their own homework. This is exactly how the whole system works to continually deflect responsibility for continual failure away from the people at the top.

We’ve been here so many times before and we’ll be back again with hundreds suffering, many dying, parents grieving and staff walking away unable to bear it any longer.

We need a statutory public inquiry now. It’s the best chance we’ve got of finally breaking out of a decade long doom loop, which has been allowed by those at the top to engulf our local mental health services.
